Appointment requests & forms

Information you submit through appointment request forms or the site assistant is used only to contact you about scheduling and your care, and is transmitted to our front desk email. Please do not include detailed medical history, insurance numbers, or other sensitive records in web forms — we will collect anything needed through secure channels during intake.

Patient forms

The new patient intake form and the peptide & GLP-1 questionnaire on this site work differently from the appointment request forms above: they do not transmit anything. Everything you type stays in your own browser. When you finish, the form assembles your answers into a summary that you print, save as a PDF, or download — you decide how and when it reaches us. Your progress is stored on your device only if you switch that option on, and the "Erase my answers" button removes it.

Analytics

This site may use privacy-friendly, cookieless analytics to understand aggregate site usage. We do not load analytics, advertising, or session-recording scripts on the patient form pages. We do not sell visitor information.

Emergencies

If you are experiencing a medical emergency, call 911 or go to the nearest emergency room. This website and its assistant are not monitored in real time.
